• Both in-person appointments and Telemedicine visits via a secure telemedicine platform are available. I am currently offering in-person sessions on Mondays.
• I am authorized to prescribe all medications, including all classes of controlled substances. However, I do not provide treatment that requires the routine prescription of controlled substances.
• All sessions will be billed prior to the session. No shows, cancellations or rescheduling with less than 24 hours’ notice (not including weekends or holidays) will be charged fully.
• All fees will be subject to an annual 2.5% increase on January 1st to adjust for inflation.
• I currently accept Mount Sinai Top tier insurance and I am out of network with all other insurance providers. However, I can provide a bill that you can give to your insurance company to request reimbursement. Please contact your insurance company directly to inquire about your out-of-network benefits.
• I offer sliding scale rates for those who are eligible.
• I do not offer 24/7 emergency coverage. In case of an emergency, please call 911 or visit the nearest emergency room. Once you are in a safe location, either you or the emergency room physician can contact me to coordinate further care.
